A medical report or letter is an official statement about your health written by a medical professional.

Commonly, medical reports or letters are requested for insurance providers, adoption and fostering, DVLA and benefit appeals.

Most medical reports are not covered by the NHS. As such, we charge a fee to cover the costs of the time our GPs and administrators spend completing these requests (please find a full list of charges here). We review these charges from time to time, so they may change.

Please note that you do not need a doctor’s letter for a housing application. You need to complete a self-assessment form which is only available online here or directly from the council. The council may contact us for information if it’s needed.

Who is this for? We can write a medical report or letter for any patient registered with us. If the request comes from the patient’s relative or another third party, we may need to seek consent from the patient before issuing the letter.

What to expect Once we receive your request we’ll be in touch with more information about how long it’ll take to write your report and any other information we may need. Due to the high volume of requests we receive, and the time to prepare reports, your request may take a number of weeks to be completed.

Covered by NHS? Most medical reports are not covered by the NHS. If you’re unable to pay for a medical report or letter, please let our team know and we can discuss alternative options. You can also access your medical record free of charge through Patient Access or the NHS App.
